We understand that:

☐ Employees may enroll, change, or discontinue contributions at any time according to company payroll procedures.
☐ Heart of Sumter will provide promotional materials and enrollment forms.
☐ Our payroll or HR department will coordinate implementation of payroll deductions.
☐ Participation does not create a financial obligation for the employer beyond administering employee-authorized payroll deductions.
